About this form​

​​Completion of this pre-enrollment form lets the childcare program know that parents have been accepted into Clarendon and are interested in using the childcare program.

Upon acceptance into Clarendon by the San Francisco Unified School District and depending on space availability, children are enrolled in the childcare program.

​

Spaces in the childcare program are filled on a first-come, first-served basis with priority given to siblings of currently enrolled childcare children.
