Enrollment Forms

DO NOT FILL OUT THESE FORMS UNTIL YOU RECEIVE CONFIRMATION THAT YOUR CHILD IS ACCEPTED INTO THE PROGRAM

 

Once you have received confirmation that there is space in Childcare for your child/chidren, you must fill out and return these forms to Childcare before your child is able to participate. Once you have filled out the forms you have three options to return them: email them, give them to the school office for Childcare or hand deliver them to Ben Pope.
